To pay from a Donor Advised Fund, Charitable Trust or IRA:

If you contribute from a Donor Advised Fund, a Charitable Trust or from your IRA, please have the fund manager or
trustee mail the check to UUFSMA, at 220 N. Zapata Hwy, Suite 11, Box 401K, Laredo, TX 78043. If your fund
manager or trustee sends a check directly to you, follow the "CHECK" instructions above.
When you initiate these payments, please notify the Treasurer in the form bellow.
